Applicant Forms

If you are applying to an ELCA-affiliated Bible camp located in Minnesota, North Dakota, South Dakota or Wisconsin and have been instructed to complete background check paperwork as part of your application, please do the following:

1) Click here to download Formatta Filler, which will enable you to open the file you'll need to proceed with the background check paperwork.  When you download Formatta Filler, a registration form will appear.  THE REGISTRATION FORM IS NOT A BACKGROUND CHECK FORM.  After completing the registration form, PLEASE PROCEED TO STEP 2 BELOW TO COMPLETE THE BACKGROUND CHECK PAPERWORK.

2) Once you've downloaded Formatta Filler, click here to access the background check forms.  Please read and complete all 6 pages, paying particular attention to the instructions at the bottom of page 6.

You are strongly encouraged to use the Formatta forms as instructed in steps 1 and 2 above.  However, if for some reason you have difficulty downloading Formatta Filler or the Formatta forms, click here for the same forms in PDF format.  The forms in PDF format will need to be completed by hand and mailed to the camp to which you're applying.  For a list of camp addresses, click here.
